A 16 year old girl presented with complaints of diminution of vision in the right eye for distance since 4 years, insidious in onset, painless and gradually progressive with a recent history of change in power of glasses. There was no history of trauma, any systemic illness or similar complaints in family.On examination the BCVA in RE was 6/18 & LE 6/6. Slit lamp biomicroscopy of right eye revealed two band lesions in the cornea at the level of endothelium better appreciated on retroillumination suggestive of posterior polymorphous corneal dystrophy (PPCD). Pentacam revealed features suggestive of ectasia. Confocal and spectral microscopy revealed epithelialisation of endothelium. PPCD is a rare endothelial corneal dystrophy identified by careful slit lamp examination. It can be associated with keratoconus, glaucoma, alport syndrome, peripheral anterior synechiae among others. Treatment is warranted in advanced cases of corneal edema. Refractive surgeries like LASIK should be avoided.
