Dr. Archana M, Dr. S Bala Murugan, Dr. Fredrick Mouttapa

PURPOSE:Posterior Cataract Opacification with vascularisation is a vision threatening challenge with high risk of bleed if taken up for Nd-Yag capsulotomy. In diabetics with vascularised pco, anti-vegf followed by Nd-Yag capsulectomy are planned. Treating a non diabetic patient with PCO with vascularisation is daunting.

MATERIALS AND METHODS:A 61year old hypertensive lady with cataract surgery done 10 years ago had painless gradual diminution of vision in RE for 2 months with BCVA 6/36.She had updrawn pupil with fibrous pco and vascularisation.RE b-scan was normal. She underwent pupilloplasty+central membranectomy_+ anti-vegf procedure under local anaesthesia.

RESULTS:On postoperative day 1, under antibiotic, steroids and cycloplegics coverage, the BCVA improved to 6/9 which was maintained until 6 months

CONCLUSION:Surgical treatment of pco has got a discrete role in managing complicated after cataracts with proper precautions
