FP868 : Hepatitis, Retinitis and An Elusive Etiologic Agent-Case Report of Co- Infection By 2 Viruses

A 28 year old male patient presented with DOV OU since 20 days.Fever and jaundice with raised liver enzymes 6 weeks prior.BCVA OD 20/2400,OS-20/600;RAPD OD. OU- AC cells 1+,Vitritis 2+,Areas of retinal necrosis with surrounding hemorrhages and disc edema OU. Negative for TB, typhoid, Malaria & syphilis.HBC,HCV,Dengue,Chikungunya,WNV,HSV,HIV and CMV were –ve in blood and serum samples.Treated with intravitreal bevacizumab OU. At 1 month,retinal necrosis increased.VA OD- 20/20000, OS- 20/1200. AC cells 2+,vitreous cells 3+ OU. Prednisolone eye drops & Oral Steroids started. In 8 days, OD- HMCF, OS- 20/1200, vitritis 4+ OU and no AC cells. Urine samples tested positive for dengue& chikungunya viruses by RT- PCR. Started on Oral Valgancyclovir 450 mg BD prescribed for a month. In 1 week, Vitritis resolved,areas of retinal necrosis rarefied. Vision OD improved to 20/1200 & OS remained stable.The patient completed one month of oral valgancyclovir. VA OD- 20/240 and OS- 20/720.
