T-ACM: Trocar Anterior Chamber Maintainer: A modified technique of infusion that uses the trocar cannula as an anterior chamber maintainer(ACM). Although routinely used in posterior segment surgery, trocar cannula is sparingly used in complex anterior segment procedures. The T-ACM creates a transconjunctival biplanar self sealing wound of appropriate size overcoming the shortcomings of an ACM like spontaneous extrusion and forced introduction into the eye due to variability in the size of corneal paracentesis incision. In this method constant infusion through T-ACM is used to maintain positive intraocular pressure during an intraocular surgery through a self-sealing sclerostomy incision at the limbus. With the use of TACM of 25G size, anterior segment surgeries like glued IOL, corneal tear repairs, and complicated cataract surgeries with posterior capsular rupture will have a stable anterior chamber. It can also be utilized in endothelial transplantation procedures like PDEK or DMEK.
