VT123 – DON technique to manage hard nuclei in Temporal SICS

Temporal SICS has advantage of not affecting corneal astigmatism to any significant effect. Hence larger incisions can be used to deliver out hard,bulky nuclei with maximum tissue respect especially to endothelium .This is important because hard black or brown cataracts are usually seen in age group above 70 yrs,diabetes where endothelium is already compromised.Large incisions of about 7mm if taken in superior SICS may induce and aggravate against the rule astigmatism.Since temporal SICS lacks advantage of traction of superior rectus muscle as seen in superior SICS, bulky nuclei delivery is challenging. This video will show “DON”(i.e.=Dial Out Nuclei) technique wherein left hand uses iris repositer to press scleral incision and dialer in right hand dials out nucleus .Advantage is =no special instruments or higher end phaco machine is required,induced astigmatism is negligible,minimal endothelial damage,any grade of hard nucleus can be delivered out.
