A 60 yr female with complaints of le eye pain for 1 year, with similar complaints in the past in be on irregular treatment on&off with multiple ophthalmologists over 2 years. on examination re healed peripheral ulcerative keratitis scars from 9 to 3 o’ clock position, similar active inflammatory process in left eye with corneal thinning seen on slit lamp. relevant investigations done & a diagnosis of le active mooren’s ulcer. she was put on systemic&topical high dose steroid with preservative free lubricants&topical 0.05% cyclosporin eye drops bd for le&followed up for 2 weeks, later with no signs of improvement was started on 7.5 mg of methotrexate per oral weekly once with supplementation with folinic acid daily in addition to the initial therapy. after 4 weeks she was underwent left eye multi layering of the peripheral cornea with amniotic membrane&a scleral barrier patch graft on the inferior 180 degree limbus. she showed good improvement in signs&symptoms&is on f/u s/p 4 months.
