To demonstrate that a posterior capsulorrhexis can also be done in an eye which has sustained a rent in a patient with a posterior polar cataract a 36 year old man came to us with visual symptoms and a visual acuity of 6/12. examination revealed bilateral posterior polar cataracts with a normal fundus.under local anaesthesia a 5mm rhexis was done and phaco surgery started under low parameters. with careful dilineation the nucleas was removed. a posterior dehiscence was noted on removal of the epinucleas. though the capsule was friable an attempt was made to achieve a posterior rhexis using a paediatric capsullorhexis forceps after an anterior vitrectomy with the use of an endo-illuminator. a 3 piece foldable acrylic lens was then inserted in the bag. results his bcva was 6/6 at one month. this technique demonstrates that a rhexis must and can be attempted in even the most trying of circumstances to ensure good out comes
