VT177 – Deep Anterior Lamellar Keratoplasty (DALK): Techniques and Challenges

DALK is a challenging procedure with a long learning curve, but due to its obvious advantages of preserving the host endothelium and maintaining anterior chamber integrity, it is being performed as a procedure of choice in those clinical conditions where endothelium is not involved by the pathology. We perform Deep Anterior Lamellar Keratoplasty in different clinical conditions. Recorded surgical videos were reviewed and analysed. Keratoconus, corneal scars and corneal dystrophies were the most common indications. Basics described in the video include concept of big bubble, types of big bubble, basic steps of DALK surgery etc. We also discuss challenges encountered during big bubble formation and stromal layer dissection and also possible ways to prevent and resolve complications. With the help of these techniques one will be benefited in tackling the challenging steps of the DALK surgery with relative ease!
