BACKGROUND: Traumatic cataract with injuries at various tissue levels and late reporting are challenging for anterior segment surgeons. In this video we will be demonstrating neglected & complicated traumatic cataract cases who reported from outreach department to our ocular trauma services. METHOD: Repair of anterior segment for restoration of anatomy of eyeball & to give sufficient time for eye to settle in terms of infection & inflammation for second stage surgery. Surgically absorbed nucleus or remnant nucleus with tough membranous cataract is really challenging with or without uveal tissue /capsule-zonular apparatus management by vitrectomy cutter or other instrumentation. We deal with co-morbid conditions separately. The approach is anterior or posterior i.e. pars plana. CONCLUSION: The challenge do lies in managing these complicated neglected cases when associated with difficult comorbid conditions to give a useful vision but with modern instrumentation.
