It’s a proven fact that Manual SICS has SIA more than Phaco, but when the same surgery is performed with incision on steep axis, the SIA neutralizes the pre-existing corneal astigmatism, thus improving the refractive results of the surgery. Results of 100 cases, operated on steep axis were analysed. All grades of nuclear sclerosis were included. Incision was placed on steep axis and size of incision was decided by expected size of nucleus and preexisting corneal astigmatism. On Post-Op. Day 1, 77 patients had UCVA of 6/18 or better, and 97 had BCVA of 6/18 or better. Three weeks post-op, 87 had UCVA of 6/18 or better, and 100 had BCVA better or equal to 6/18. Residual Astigmatism at 3 weeks was less than or equal to 1.25 in 73 patients, 19 patients had 1.5 to 2.0 dioptres of residual astigmatism, and 8 patients had residual astigmatism more than 2.25. MSICS, in our study, has been able to achieve good refractive outcomes in terms of residual refractive error and patient satisfaction.
