A 64 years old male,with high hypermetropia and senile immature cataract(SIMC) in both eyes presented to us. He had a shallow anterior chamber with raised intraocular pressure(IOP), gonioscopy showing narrow angles and axial length being 15.2mm in both eyes, bringing us to the diagnosis of Bilateral Nanophthalmic Glaucoma with SIMC. He was started on antiglaucoma drugs and laser peripheral iridotomy was done in both eyes. On follow up, the IOP was controlled and iridotomies were patent. Dilated fundus examination showed a cup disc ratio of 0.3, but 360ْ serous choroidal detachment was noted in both eyes. In view of significant cataract,4 quadrant scleral windows were done in both eyes 2 weeks apart, following which choroidals decreased. The patient was advised to undergo elective cataract surgery at a minimum period of 6 weeks from the date of scleral window. He then underwent uneventful cataract surgery in his right eye. The patient is doing well now, with stable IOP and good vision.
