A 54 years old female, a known case of Cytomegalovirus(CMV) Anterior Uveitis and Secondary glaucoma, with good intraocular pressure(IOP) control, on antiglaucoma drugs since 10 years, presented to us.Both eyes anterior segment was quiet and fundii showed a cup disc ratio of 0.5.She followed up regularly for 3 years since then, during which, she had recurrent attacks of anterior uveitis and raised IOP in either eye, which were controlled by medication.In the last 6 months, the IOP in both eyes continued to be intractably high,inspite of maximum therapy.Hence, right eye trabeculectomy was 1st undertaken.3 months later, the left eye surgery was also done,IOP being persistently high, a pars plana vitrectomy along with a trabeculectomy was done, to avoid any vision threatening complications.6 months postoperatively the patient is doing well, with good IOP control, stable vision and fundii in both eyes. Secondary glaucoma in CMV uveitis is a challenge, which can be managed effectively.
