A 65 years old male,presented with a painful red eye on his left side.His vision in the right eye was 6/6 and left eye was counting fingers close to face.Both eyes had a shallow anterior chamber,the left eye having corneal edema with a mid dilated fixed pupil.Intraocular pressure(IOP) was stable in both eyes,as he was already on antiglaucoma drugs.Gonioscopy showed occludable angles in the right eye and the left eye could not be seen due to a hazy view. Both eyes iridotomies were done and antiglaucoma drugs continued.He returned after 1 week with pain, redness and nausea. Iridotomy was not patent and IOP was 50 mm Hg.He was started on intravenous mannitol and a repeat iridotomy was done,but IOP was uncontrolled.B scan showed multiple intraocular pockets of aqueous.A diagnosis of Malignant Glaucoma was made and the patient underwent a pars plana vitrectomy with lens extraction, following which the IOP was well controlled,without any antiglaucoma therapy.
