Introduction – A case series of 12 cases of limited posterior segment inflammation post cataract surgery.
Material and methods – 12 cases of uneventful phacoemulsification developing a mild anterior segment but severe posterior segment inflammation at Day2/3. In all cases there was no pain/lid edema/discharge, Snellen’s acuity 4/60-1/60, few KP’s, Gd-1 flare/cells,PCIOL in bag,fundus exam- media haze.USG B-scan – vitreous opacities.Alongwith topical steroids and cycloplegics,instituted oral steroids (1.5mg/KgBW).Results-Initially static but gradual improvement with good recovery at 06wks(BCVA-6/6-6/9 Snellens).
Conclusions – This setting of minimal anterior segment but severe posterior segment inflammation was at variance from acute infectious endophthalmitis.This could be attributed to intraoperative diffusion of inciting agents through the zonular apparatus into vitreouscavity(inj.Adrenaline/pilocarpine,butadiene,preservatives).The ant segment spared because of rapid aqueous turnover.
