24 years old male while making design on POP ceiling, fell down from a height of 10 feet and had left fronto-temporal contusion. CT brain was suggestive of left fronto-parietal subdural haematoma ,subarachnoid haemorrhage with midline shift.

He underwent left fronto-temporo-parietal craniotomy, evacuation of subdural haematoma and lax duraplasty same Day. 40 days later , he reported with diminution of field of vision on left side. His distant vision in right eye was 6/6 and in left eye 6/36. By confrontation test, he was found to have loss of upper half of field of vision on left side. Ultrasonography revealed vitreous haemorrhage in the left eye. Trans pars plana vitrectomy was done in the left eye and postoperatively patient vision improved to 6/9 in the left eye.

We hereby report a case of Terson syndrome after head injury, having subarachnoid haemorrhage associated with vitreous haemorrhage in left eye and visual acuity improved to normal after vitrectomy.
