Purpose: To compare results of hang-back and conventional recession surgery in large angle exotropia.

Methods: A randomized, clinical trial, including 20 patients with exotropia was done.10 patients underwent conventional surgery , in rest of the cases one muscle was put on hangback . A single surgeon performed all surgeries.

Results : Exodeviation improved from 50 ±10 PD (Δ) preoperatively to 6±6 Δ postoperatively for hang-back recession (n=1 0) and from 50 ±12 Δ to 10±6 Δ for conventional recession (n=10). Mean follow up was 6 months.

Conclusion: Hang-back recession is a better alternative to conventional recession in large angle exotropia.
