A 55 yr old lady presented with acute red eye following injury with a thorny bush 5 days duration. Despite multiple attempts at various centres the thorns were still embedded in the corneal stroma and subconjunctivally leading to severe granuloma formation pain and gross diminution of vision.
The major challenges were not only prevention of fungal infection due to vegetative injury but also to control the severe inflammation. Finally removal of deeply embedded foreign bodies and improvising surgical technique to restore the vision and provide symptomatic relief to patient. Finally the patient became asymptomatic and regained 6/6 vision. In conclusion-meticulous planning systematic approach and surgical innovation helped in treating a difficult case of trauma with good result
