VT101 – Impossible means I AM POSSIBLE (Managing challenging situations)

Dr.O P Agrawal

In the life of a phaco surgeon many a times, comes a situation when it appears almost impossible to successfully do a lens implantation. One such challenging situation was a post traumatic case with absorbed cortex, posterior synaechia and thick fibrous and calcified membrane. It was challenging situation as there was all likely hood of posterior capsule tear while removing the new membrane and subsequent difficulty in lens implantation. To rule out any posterior segment pathology a B scan was done, which was normal. The options were to remove the membrane either by anterior route or by doing a pars plana Vitrectomy. I chose the anterior route because it gave me a slender chance to save the posterior capsule and put the lens in the bag. The Various steps of managing the case are shown in the video clearly. The membrane was successfully removed and I could achieve an “in the bag lens implantation” This was definitely one of the “HOW to WOW” case.
