The video highlights a rare case of Alports Syndrome (AS) that presented to ophthalmology department of a tertiary care eye centre with diminution of vision. The detailed ophthalmic evaluation revealed an anterior lenticonus with posterior lenticonus along with posterior subcapsular opacities. The systemic examination revealed sensorineural deafness and investigations revealed microscopic hematuria. Histopathology confirmed diagnosis of AS. Phacoemulsification with posterior chamber intraocular lens was decided and performed with all the due precautions. The video highlights the precautions to be taken during capsulorhexis(CCC), hydrodissection and during phacoemulsification so as to avoid the complications like flower petal pattern of CCC and posterior capsule rupture. It will also highlight the measures to be taken when these complications occur and the best possible solution to deal with the situation.
