VT181 – All is well that ends well

Purpose To place the IOL in a secured way to maintain its centration and stability in a subluxated cataratous lens case. To come out safely from the unexpected challenging situation.

Methods Subluxated cataract case is planned to manage by using capsule retractors and cionni’s ring. Cionni’s ring got broken while inserting through small incision, it was replaced after enlarging the corneal wound. Next, prolene suture attached to the haptic of cionni’s ring while tying to the sclera, got excessively pulled to produce zonulodialysis on the opposite side. In the event there occurred posterior capsule rent. New prolene suture was passed from the opposite side and gently tied to sclera. A multipiece IOL is then placed in the sulcus. IOL optic is captured into the intact capsulorhexis to avoid its dislocation. Results After 15 months of postoperative course, IOL is stable, well centered and secure.

Conclusions All is well that ends well.
