Internal limiting membrane peeling (ILM) has definitely improved the success of macular hole surgery. ILM peeling in scenarios other that idiopathic age related holes may be tricky. We present videos of two such situations in which ILM peeling may be tougher. The first video is a case of high pathological myopia with a retinal detachment and macular hole. Here the challenges are peeling the ILM in a detached retina and also the added difficulties of operating in an eye with pathological myopia. The other video demonstrates peeling of an epiretinal membrane and internal limiting membrane in a case of branch retinal vein occlusion. Intra-operative OCT in such cases can help confirm the correct plane between the epiretinal membrane and the retina. Also peeling an ILM in an oedematous retina poses its challenges as shown in the video.
