A 9 year old female child presented to our hospital with defective vision in left eye. Her visual acuity was counting fingers close to the face in left eye. Early posterior subcapsular cataract and an old vitreous haemorrhage with a persistent fetal vasculature stalk were noted in the left eye. Left eye sutureless trans pars plana vitrectomy was planned with 27G vitrectomy system. Infusion and two ports were secured. A stalk arising from the disc and dense VH were noted. VH was cleared and stalk was cut close to the disc and cauterized. Blood clot at the disc was trimmed, with minimal vitreous base shaving. Visual axis was made clear. No bleeders seen. Fluid-air exchange was done and ports were removed. No leak was noted from sclerotomies. At 1 month follow up, disc edema with residual stalk was noted along with early PSCC in lens. Visual acuity in left eye was 6/60 with BCVA 6/18. Steroids were tapered and spectacles prescribed. At final follow up at 1 year, BCVA in left eye was 6/12.
