A 48 year female presented with poor visual gain following right eye cataract surgery done elsewhere. She had undergone bilateral Radial Keratotomy 22 years back. Uncorrected visual acuity (UCVA) was counting finger at 2 metre right eye (OD) and best corrected visual acuity (BCVA) of 6/12 with +12.50 Dioptres of hypermetropia. Patient underwent IOL explantation through a small scleral tunnel after cutting the IOL in two segments and implantation of +32D single piece acrylic IOL in the bag. Postoperatively, UCVA improved to 6/18 and anterior chamber was well formed without any wound gape. BCVA after 10 days was 6/9 with +1.5 dioptre cylinder.
