Knowledge of the site and distance of extra ocular muscle insertion is very helpful in preoperative planning of strabismus surgery.AS OCT is a new tool which can accurately detect horizontal rectus insertion and measure the distance of the insertion from the limbus. In this study AS OCT – Visante model is used in complicated squint like Strabismus reoperations where previous reports are not available, and pathological conditions like Thyroid myopathy, High Myopia, and trauma . The extra ocular muscle insertion is identified as the end of the cleft between the rectus muscle and the sclera and is measured with the caliper function of the Visante OCT. This is documented and later intraoperative measurements are done with surgical calipers, photographed and the two readings are compared and found to be within 1 mm range in 90% of cases .Anterior Seg OCT is a new tool to enable the surgeon a better pre operative planning and accurate outcome in complicated strabismus
