Purpose: To evaluate visual, refractive and topographic outcomes of transepithelial phototherapeutic keratectomy(t-PTK) followed by accelerated collagen cross linking(KXL) versus KXL alone for moderate keratoconus(KC).

Methods: Non randomized controlled trial was conducted in which 40 eyes with moderate KC underwent topography guided t-PTK followed by KXL(group 1)and 40 eyes underwent KXL only(group 2). Visual, refractive and topographic outcomes were evaluated at 1,6 and 12 months postoperatively. Results: In Group 1,mean uncorrected distance visual acuity and mean corrected distance visual acuity improved significantly (p<0.01),reduction in mean spherical equivalent noted(p<.001) and topography showed significant flattening(p<0.05).In group 2,no significant difference was observed in any parameter.

Conclusion: As compared to KXL alone, combined topography guided t-PTK and KXL results in better visual, refractive and topographic outcomes in moderate KC and avoids corneal transplantation
