Purpose: To describe the atypical clinical, OCT and angiographic findings in a case of unilateral acute posterior multifocal placoid pigment epitheliopathy (APMPPE).
Methods: A young female came with sudden onset blurring of vision in OD. No other systemic history. BCVA was 6/60 in OD and 6/6 in OS. OU anterior segment and OS fundus was unremarkable. OD fundus showed clear vitreous with yellowish white multiple circular lesions at the posterior pole. OCT showed cystic edema with subretinal fluid at the fovea. OD FFA showed disc leakage, early hypo with late hyperfluorescence.
Results: Patient was started on intravenous steroids. There was resolution of the SRF and placoid lesions. At 1 month FU,she had full visual recovery with complete resolution of fluid.
Conclusion: APMPPE is usually a bilateral condition, in young adults. Subretinal fluid and disc leakage has not been reported very often in APMPPE. We report this case of unilateral APMPPE with SRF, cystic changes and disc leakage.
