FP1732 : Novel Surgical Technique in the Management of Central Retinal Artery Occlusion

Dr. Vinay Prasad, P11758, Dr. Nishikant Borse

CRAO patients typically present with sudden, severe, painless loss of vision. In this case series of 10 cases of CRAO , the presenting visual acuity ranged from HMCF to CF 4ft. Patients already had undergone conservative treatment before.

Since conservative approaches in such cases seldom yield good results, it was decided to perform a low IOP vitrectomy using 23 G PPV and FGE followed by central retinal artery sheathotomy and ILM peeling over the optic nerve helping to relieve the kinking and blockage at level of central retinal artery.

Following this a low IOP was maintained post operatively to allow for recanalisation and reperfusion of the inner retinal layers.Follow up visits included Visual acuity testing,IOP monitoring , fundus photography and OCT macula.Post operatively at 6 weeks follow up average visual acuity improved with most patients gaining vision upto 6/120. To conclude , this surgical technique offers a better chance to such patients over conservative management
