FP1785 : Watermark on Anterior Capsule Following Profuse Irrigation Through the Central Hole of an ICL

Purpose:A grey staining of the anterior capsule was noticed during forced irrigation through the central hole of an ICL with the centraflow. A thorough irrigation through the central hole is recommended to remove the visco-elastic from under the ICL so that it does not clog this opening later resulting in raised post-op IOP, as there are now no co-existing PI’s. We observed this grey mark in several cases and referred to it as the ‘Watermark on the anterior capsule’

Methods :We used a bimanual irrigation aspiration cannula for visco removal. One opening of the irrigation tip is placed over the central hole. The second opening, is covered with the shaft of the aspiration cannula. This forces irrigating BSS down the central hole and in the space between the ICL and the crystalline lens. A fluid wave is seen to spread under the ICL. A watermark on the anterior capsule seems to develop when irrigation is done for over 15 seconds and fades away in 40 seconds.
