Introduction : Optic nerve aplasia ( ONA) is a rare developmental anomaly characterized by congenital absence of optic nerve ,retinal blood vessels and retinal ganglion cells. Till date 38 cases of congenital ONA have been reported in literature . Acquired ONA has never been reported.
Case Report : 75 year old presented with complaints of loss of vision in his left eye which was associated with contralateral hemiparesis and aphasia since 2 years . Hemiparesis partially recovered over course of 2 years but vision didn’t improve . Complete ophthalmological and systemic evaluation was done which revealed ONA and internal carotid artery thrombosis .
Conclusion : Optic Atrophy is usual presentation following ICA thrombosis and not ONA .Acquired ONA has never ever been reported .To prevent occurrence of similar attack in future in other eye systemic evaluation is necesssary .
