Introduction : A one eyed patient with corneal decompensation, congenital glaucoma, dislocated IOL was managed successfully with a quadruple procedure of corneal transplant, IOL explantation, secondary Iris claw IOL and glaucoma filtration shunt surgery

Methods :- A one eyed patient of congenital glaucoma had undergone trabeculectomy with IOL implantation at some other centre. However he had posterior capsular tear and IOL disclocation leading to corneal decompensation. His IOP also was raised and uncontrolled with medical therapy. His vision was CF 1 m and IOP 36. We perfomed a quadruple procedure involving IOL explantation, sec Iris Claw lens implantation, Penetrating keratoplasty and glaucoma filtration device introduction

Results: His vision improved to 6/12(p) BCVA and IOP was 12mm 3 months postoperative

Discussion : In this paper we discuss how a quadruple procedure can give successful outcomes if done meticulously with correct technique and understanding of the case
