A 22-yr-old boy presented with heaviness in right eye(RE) and redness in left eye(LE) for two weeks.Visual acuity was 6/6 RE and 3/60 LE.Torchlight exam showed facial asymmetry with atrophy of left side and deformed left orbit.Left enophthalmos,facial palsy and pseudotosis was present.Slitlamp revealed Fuch’s heterochromic iridocyclitis in RE while LE showed vascularized corneal opacity.RE fundus showed hypotonic maculopathy & was obscured in LE.Ocular motility was normal.Intraocular pressure was 5mmhg in RE and 12 mmhg in LE.Left upper and lower limbs were atrophic.Literature reviewed & diagnosis of PR syndrome made.CT scan supported the diagnosis.Topical
Difluprednate in RE and Moxifloxacin in LE started.Referral for UBM/silicone oil injection was advised.PR syndrome affects skin,soft
tissue & bones of one half of face.Eye involvement occurs in 10–35%.
Conclusion:PR syndrome is rare and severe bilateral eye affection is rarer.Both eyes need follow up for development of abnormalities.
