25yr old female presented with acute anterior uveitis, enophthalmous, ptosis, miosis, heterochomia of iris. There was hemiatrophy of face, limitation of ocular movements superiorly and laterally. Upper lip mouth and nose were shrunken. It is a variant of localized scleroderma. Mild ptosis was present with absence of eye lashes. There was ciliary congetion with lost iris pattern. The intraocular pressure was normal in both eyes. The v/a in left eye was 6/12 with pinhole improved to 6/6. The right side of face and right eye was normal with v/a 6/6. Topical prednisolone acetate, and cycloplegics with systemic corticosteroid was helpful to alleviate the eye complains. A connective tissue disorder or autoimmune disorder suspected. Initial facial changes usually involve the area of the face covered by the temporalis or buccinator muscles. progressively spreads from the initial location, resulting in atrophy of the skin and its adnexa, as well as underlying subcutaneous structures
