Materials &methods–Prospective analysis of a case of inadvertent anterior chamber(AC) perforation with injection of anesthetic into AC during peribulbar anesthesia. Patient managed medically & data regarding vision and ocular findings upto6 months was collected.
Results–28year old male patient with nystagmus both eyes had best corrected vision of 6/12 prior to trauma. 1 day after event his vision was 1/60. He had total corneal epithelial defect, stromal edema, shallow AC, perforation site at superior limbus. Bscan normal. He was treated with topical steroids, antibiotics, lubricants, oral vitaminC &antioxidants. At 6months he had superficial punctuate keratitis, clear stroma, optic disc pallor, best corrected vision 6/36.
Discussion–To date posterior segment perforation has been reported during peribulbar anesthesia & to best of our knowledge, this is first case report of AC perforation. It has guarded visual prognosis due to toxicity of drug on ocular surface & optic nerve.
