Descemet membrane detachment in DALK can lead to a double anterior chamber and compromise the vision and outcome of the surgery. Herein, we present the intra-operative events that can pre- dispose to this complication. The management of a detached Descemet membrane can be conservative or air injection/Descemetopexy. The video describes the technique of descemetopexy and outcomes of the cases with both the management strategies.
