35 year old male with 3 months status post parsplana vitrectomy and lensectomy with traumatic iridodialysis and mydriasis in left eye was taken up for secondary PCIOL implantation and repair of iridodialysis and mydriasis under peribulbour anaesthesia. After making two scleral flaps 180 degree apart, scleral fixation 3 piece foldable IOL was done using a bent 20 G needle and a microrhexis forceps and both the heptics were externalized and embeded in the sclera . Iridodialysis and mydriasis was repaired subsequently using 10-0 straight needle prolein sutures. Residual viscoelastic substances were removed and corneal wounds were closed with stromal hydration. The scleral flaps and conjunctiva were apposed using 10-0 mono filament nylon sutures and eye was patched overnight after putting antibiotic and steroid drops. After the initial spike of inflammatory reaction, corneal edema and increased IOP were taken care of, patient enjoyed BCVA of 6/9 at the end of 1month with quiet eye.
