VT30 – Glue less intrascleral haptic fixation of PCIOL with DSEK in Aphakic bullous keratopathy

Purpose: To demonstrate a novel surgical technique of IOL fixation with DSEK in Aphakic bullous keratopathy (ABK).

Method: A 70 years old male had cataract surgery 4 years ago and was left aphakic. Had features of ABK for 1 year.

Results: Scleral flaps and scleral pockets were made 180° apart, 1.5 mm from limbus. 5 mm main incision was made superiorly. Both leading and trailing haptics of Multipiece rigid IOL were exteriorized 180º apart under the scleral flaps with the help of SFIOL forcep and placed in the scleral pockets. DSEK procedure was subsequently performed including descemetorrhexis and sheet glide assisted lenticule implantation. Postoperatively, a clear cornea was achieved at 3 months and visual acuity as 6/18 with centered PCIOL.

Conclusion: Intrascleral fixation of a PCIOL combined with DSEK is a safe, effective method to ABK. The IOL fixation was strong enough to sustain the manipulation for corneal procedures.
