After cataract or posterior capsular opacification (PCO) is a gradual clouding of the lens capsule which causes disturbance in vision after cataract surgery. It is the most common complication following surgery which can be easily treated with Nd:YAG posterior capsulotomy. We present a video of a young male who was operated at our institute for pediatric cataract and presented years later with a tough PCO which could not be resolved with membranectomy also. This patient’s intraocular lens was exchanged years later and the patient regained good visual acuity. This video shows that with timely use of proper vitreoretinal techniques, excellent visual outcome can be achieved in complicated cataract surgeries.
