Aim : To report an uncommon differential diagnosis of conjunctival cyst. 49 years old male presented with painless progressive swelling in right lower lid since 1 year. BCVA in both eyes was 6/6. On eversion of the lower lid a 4mm x 5mm non tender, transilluminating subconjunctival cyst with bluish tinge and mobile yellowish aggregate was seen. History of pork consumption was present. It was suspected to be a parasitic cyst with dead scolex. Full thickness right lower lid mass excision biopsy and histopathological examination was done. It revealed sebaceous ductal cyst with luminal keratin.
Discussion: Conjunctival cysts can be of varied etiology, one of which being sebaceous ductal cyst. While sebaceous ductal cyst (true cyst) may mimic a parasitic cyst (pseudo cyst) clinically, histopathological features are different and help in the final diagnosis.
