A 35 year old patient presented with complaints of recurrent swelling in the upper and lower lids on both eyes since 8 yr.On examination there were 6-8 cystic painless swelling of 0.2-1cm occupying right upper and lower eyelid, 3 swelling involving the left upper eyelid were seen.Ophthalmic examination was otherwise normal.Peri-oral,peri-nasal,vulval examination normal.Transillumination positive.Biopsy performed under local anaesthesia.The lesion treated with radiofrequency ablation.Microscopy:Keratinizing stratified squamous epithelium with sub epithelial tissue showing a cyst wall lined by two layers of cells with prominent apical snouting.Sparse inflammatory infiltrate consisting mixed inflammatory cells & few capillaries,features consistent with apocrine hidrocystoma with no evidence of malignant change.Conclusion:Apocrine hidrocystomas should be considered in the differential diagnosis of eyelid mass.Treatment of choice: surgery or radiofrequency ablation.
