VT163 – TRAPPED GAS CAUSING MAYHEM

INTRODUCTION : Intraocular gases are used in VR surgery for intermediate duration tamponade and have several complications like raised IOP, cataract etc AIM To report a unique, previously unreported complication of C3F8.

METHODS : A 30 yr man underwent LE PPV with silicone oil for RhegRD post blunt trauma. Phaco with PCIOL done for traumatic cataract 10yrs back. IOL was inferiorly subluxated. IOP was high post PPV for which silicone oil was exchanged with C3F8 which persisted for more than 6 weeks during which IOP controlled medically which spiked after 6 weeks with persistent gas for which removal planned. Intraop gas was present in AC with inf dislocated IOL with redetachmentand was explanted,RD settled an Si oil injected after which IOP normalised. We hypothesise C3F8 gained entry between the iris and IOL through zonular dehiscence where it got trapped and dislocated the IOL. Presence of gas precluded preop detection.

CONCLUSION :  C3F8 can cause IOL dislocation with compromised zonules.
