Causes of Zonular Dialysis are Marfan’s Syndrome, Homocystinuria, Weil-Marchesani syndrome, Hyperlysinemia, Uveitis, Hypermature Cataracts, and Pseudoexfoliation syndrome, but most common cause is trauma. It complicates lens surgery eg. vitreous loss, nucleus subluxation, postoperative complications such as decentration of the IOL. However, new devices that minimize stress on compromised Zonules during and after surgery decrease the risk of complications. This video shows use of Endocapsular ring (ECR), Cionni’s ring (CR) and Hofmann’s pocket for extraction of Subluxated Cataract and IOL implantation. Use of Cionni’s ring and suturing to Sclera gives better support to bag and prevents future chances of subluxation of PCIOL with the bag in vitreous. In this case, we have achieved good post-op visual acuity with well centered IOL and stable bag.
