Cataracts with post.capsular palque & post.subcapsular fibrous thickening are the challenging cases for all Cat surgeons.Management of post.capsular plaque & post.subcapsular fibrous thickening r done in 3 different ways depending upon type of capsular opacity.1 Coaxial irrigation & aspiration hand piece used with regular setting (as used for cortical removal ) for fibers attached under anterior caspule as well as on post.capsule.Post. capsule is polished in such a way that every time IA hand piece kisses post.capsule & remove fibers.2 Hydro & mechanical separation of fibrous plaque & lens fibers done with the help of hydrocannula as well as fine iris spatula.3 Mechanical peeling & debulcking of fibrous palque opacity done with the help of capsulorrhexis forceps.All above methods require fine surgical skills with highest level of patience. An immediate postop.result shows better quality as well as quantity of vision. A long term post op. results shows reduced no of YAG Cap procedures
