Neovascular glaucoma (NVG) is a sight threatening disease, extremely difficult to manage and required multimodality treatment approach in the right time. Indian population, NVG may present with good visual acuity ranging 6/6 – 6/9.

We report 3 such cases of NVG,PDR, treated with panretinal photocoagulation(PRP) followed by intravitreal bevacizumab(IVB) (1.25mg in 0.05ml).

First case, IOP was well controlled with two topical antiglaucoma drugs (BCVA 6/9 in both eyes) following PRP and IVB. Second case needed IVB followed by trabeculectomy for IOP control (BCVA 6/18).Third case was one eyed, underwent trabeculectomy, 2 months later developed tractional peripheral retinal detachment for which vitrectomy, silicon oil injection was done Post op BCVA is 6/18 and a well functioning bleb. Neovascular glaucoma needs aggressive treatment in terms of not only laser, IVB but also timely trabeculectomy and vitrectomy in in selected cases to give satisfactory visual outcome 6/6 -6/12.
