A 35 years old female presented to our OPD with complaints of difficulty in walking for 4 months and seeing double images for 10 days. History of slurring of speech,urinary incontinence present. On examination speech dysarthric,fine touch and pain impaired over right side face and body,right hemiparesis and ataxic gait were present. Cerebellar functions impaired.
On ocular examination OU: anterior and posterior segment were normal. Extraocular movements showed complete right horizontal gaze palsy, left horizontal gaze had only abduction of left eye with ataxic nystagmus with absent right eye adduction,with normal vertical gaze. Vision OU: 6/24.
Complete blood investigations,CSF analysis,ECHO,ELISA for HIV were done and found to be normal. MRI brain with whole spinal screening showed possibility of multiple sclerosis. Confirmed diagnosis of multiple sclerosis was made. Patient was referred to physician and treated with systemic steroids.This being an interesting case, is presented here.
