Orbital pseudotumour, sclerosing type is a commonest orbital disease with rare extraorbital extensions. A 29yrs old male patient presented with pain, redness, forward bulging, reduced vision and drooping of lid of right eye since 2months. On examination: Vision-finger counting close to face, RAPD, proptosis, ptosis, absent movements seen. Fundus-disc odema present. CT and MRI brain-destructive soft tissue lesion involving paranasal sinuses with destruction of orbital walls on right side extending into anterior cranial fossa. Endonasal biopsy from maxillary sinus-Pseudotumour. Immunohistochemistry showed sclerosing inflammation. Repeat surgery was done to remove maximum mass. Patient treated with systemic steroids, there is reduction in proptosis, improvement in vision and EOM. Pseudotumour orbit, sclerosing type can have faster and extensive encroachment on to surrounding structures and can mimic neoplasm. Hence early intervention need to be done to exclude neoplasm.
