Aim: To study indications, outcome and risk factors for graft survival in optical penetrating keratoplasty (OPK) patients.Study design: Retrospective hospital based.

Method: Data of patients who underwent OPK between January 2014 and December 2015 recorded.Less than 6 month followup cases as well as therapeutic, lamellar PK and paediatric cases excluded.Surgical indications divided into good, fair and poor prognosis Graft survival period studied and relationship with risk factors analysed using Kaplan Meier survival analysis and Cox Proportional Hazard Regression analysis.

Results: Out of 89 patients, 60.6 % patients were male and 39.2% female. Median age was 56 years. Commonest indication was corneal scarring.Mean graft survival period was 8 months.Phakic and pseudophakic eyes had had better survival than aphakics (mean survival 9.3 vs 5 months).

Conclusion: Graft survival is dependent upon various factors.Paucity of followup and non compliance can be contributory in graft failure
