FP404 : Traumatic Graft Dehiscence in Dalk Post Blunt Trauma

Purpose: To evaluate outcomes of blunt trauma causing graft dehiscence after deep anterior lamellar keratoplasty (DALK).

Methods:434 cases operated for DALK from Jan’07 to Oct’15 were reviewed with reference to graft dehiscence after blunt trauma. Mode of injury, duration after surgery,BCVA and endothelial cell density were noted.

Results: 4 cases; 3 males and 1 female were reported. Mean interval between DALK and injury was 11.5months. Mode of injury was by fist in 3 and by stick in 1 case. Descemet membrane perforation with lens damage and vitreous prolapse was seen in 2 cases. All underwent repair. Mean follow up was 21.5months. All grafts were clear till last follow up with BCVA ≥6/12. Mean endothelial cell loss after trauma was 38.7%.

Conclusion:The corneal wound post DALK is considered to be strong,but a severe force may cause wound dehiscence. Following repair, graft clarity and vision is good but endothelial cell density is reduced which may affect long term graft survival.
