FP500 : Importance of Histopathological Examination in Lid Swellings- A Case of Sebaceous Cell Carcinoma

A 60 years old male presented with Right eye upper lid swelling, progressively increasing in size for 6 months. Examination revealed a pigmented lesion of size 3*3*2 mm in the medial one third of upper lid and diagnosed as wart. It was excised and sent for histopathological examination which showed it as poorly differentiated sebaceous cell carcinoma. Later It was taken for full thickness pentagon shaped lid margin excision with 4 mm clearance and sent for HPE followed by primary closure of defect. HPE showed that the excised specimen was tumour free in all margins.

Sebaceous cell carcinoma has different modes of presentation like recurrent chalazion, chronic blepharitis and lid inflammatory conditions. Hence, all swelling of lids irrespective of its size and duration should be sent for HPE after excision. This case is presented to emphasize the importance of HPE in lid swellings that helps in timely management and prevention of further metastasis that warrants destructive procedures.
