To report a rare case of isolated abscess of left Inferior rectus muscle(IRM) with unique retinal presentation.
A 60 yr male with c/o diminution of vision, redness & acute pain in LE referred to Retina OPD of a tert. eye care center with diagnosis of suspected inf. retinal detachment. On exam mild proptosis with inf. orbit fullness found, ocular motility restricted.On fundus exam, massive inf. retinal indentation seen reaching upto macula & optic disc & retinal folds over macula. Orbital CT Scan, EIA(Cysticercosis) was done.Provisional diag. of IRM abscess made.Pt was followed up with Fundus photo,OCT,Ocular sonography during the tt. Incision & drainage of pus done & IV antibiotics started.Gram staining, pus & blood culture were sent. After 1&½ mth, vision returned to normal, indentation totally subsided with normal retina.
As per literature till now only 8 cases of extraocular muscle abscess have been reported,of which only 2 were idiopathic & none of them have such presentation.
