FP812 : A Case of Phacolytic Glaucoma with Pseudoexfoliation Syndrome

A 65 years old patient presented with progressive diminution of vision, mild redness & pain of left eye of 7 days duration, for which he had taken some medications before coming.Visual acuity of left eye was finger counting positive at 1m.On examination conjunctival congestion seen.Pupil was mid-dilated & sluggish reacting, not dilated fully after application of dilating drugs.A thick hypopyon seen at the bottom of anterior chamber.Lens was opacified & small whitish material seen on anterior lens capsule.Small whitish flakes also seen on the pupillary margin & there was grade 1 aqueous cells & flare.Intra ocular pressure was 37.2 mm of hg.Gonioscopy shows trabecular pigmentation but open angle.Patient was treated by topical timolol 0.5%, prednisolone acetate1%, atropine1% & oral acetazolamide 250 mg. Small incision cataract surgery with PCIOL implantation done after lowering the IOP below 30 mm/hg with mannitol.Visual acuity improves to 6/24 & 6/12 post operatively at 1 week & 1 month.
